Job Description:
Purpose:
The School Program Manager is accountable for all school support and outreach provided and conducted by DYS.
Key responsibilities include the planning, implementation and evaluation of all school programs and services aligned with the strategic plan to fulfill the DYS mission. The School Program Manager is a public spokesperson and liaison for the organization and is directly involved with creating awareness and gathering support from the community.
The School Program Manager is responsible for identifying and directing all resources required to meet or exceed the school program goals established by the Board of Directors.
Specific Responsibilities:
1. Work with the DYS school service range to proactively provide diabetes education to the target audience.
2. Maintains a log of contacts, visits and time spent on individual cases and record in current database tracking system.
3. Develop and revise diabetes care plans where needed.
4. Travel to schools in DYS service region to implement one-on-one and group trainings and presentations. Conduct service evaluations and follow up on visits.
5. Outreach: coordinate school mailings and outreach to heath care professionals and related medical organizations and facilities.
6. Preparation and analysis of program and service reports and other reporting as directed.
7. Field phone calls and emails from constituents seeking assistance.
8. Serve as DYS point of contact for special education programs relating to diabetes education (such as the ABC's of Diabetes in the Classroom program collaboration with AADE).
